Job summary

Affinity Health Management is seeking a registered nurse to plan, organize and direct hospice care in home and community settings. The nurse will conduct assessments, develop care plans, administer treatments, and coordinate with families and the interdisciplinary team.

Responsibilities include patient and family education, documentation, on-call duties, and ensuring a safe, supportive care environment. Requires Maine RN license, CPR, and a valid driver’s license.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing to practice.
• Minimum 1 year of experience as a professional nurse within the last 3 years; or BSN with 2 years' experience in public health/home hospice preferred.
• Supervising hospice aides not required.
• Contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Prolonged walking/standing; ability to lift/transfer patients and equipment; visual/hearing acuity.
• Must be a licensed driver with insured vehicle in good order.
• CPR certification (hands-on verification required).

Responsibilities

• Plan, organize and direct hospice patient care.
• Provide nursing care and administer prescribed treatments.
• Develop and revise the care plan with patient/family involvement.
• Coordinate plan of care with physicians and interdisciplinary team.
• Educate patients and families on nursing and care needs.
• Maintain accurate clinical notes and documentation.
• Participate in on-call duties and ensure care continuity.
• Supervise hospice aides when assigned.
