Qualifikationen

• Management experience not required.
• Offer contingent upon references and criminal background checks.
• CPR certification required.

Aufgaben

• Complete initial and ongoing comprehensive patient/family assessments for hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise plan of care with patient/family input as status changes.
• Counsel patient and family; provide health instructions and assist with ADLs.
• Act as case manager as assigned and coordinate care for caseload.
• Maintain accurate clinical notes and communicate with physicians and care team.
• Participate in on-call duties and ensure equipment/assets are available.
• Supervise aides as needed and pursue ongoing professional development.
