Community Health Network is seeking a Clinical Liaison to ensure hospice patients referred to Community Hospice receive outstanding care as they transition to home hospice. Responsibilities include daily referral follow-up, coordinating meds and supplies, and ensuring seamless discharge.

The role requires an RN license in Indiana, 2 years hospice experience, and valid Indiana driver’s license. Close collaboration with Palliative and Case Management teams is essential.

Qualifications

• Graduate of NLN, CCNE, ACEN, or NLN CNEA accredited nursing school, or three years of related professional nursing experience.
• Bachelor’s degree in Nursing preferred.
• Licensed RN in Indiana (NLC).
• 2 years Hospice experience required.
• Current Indiana Driver’s License and automobile insurance.

Responsabilités

• Ensure hospice patients referred to Community Hospice receive an excellent care experience.
• Follow patient referrals daily until discharge and ensure meds and supplies are ordered prior to going home.
• Follow patient home and admit to hospice service when necessary; coordinate with admission nurse.
• Build effective relationships with hospital referral sources to increase hospice referrals.
