Job summary

Compassus is seeking a Hospice Clinical Liaison to promote our care philosophy and coordinate admissions from hospital to home. The role requires an RN license and 2–3 years of related clinical experience, with hospital/long-term care backgrounds highly valued.

The successful candidate will educate referral sources, patients, and families about hospice benefits, and work across facilities to ensure smooth admissions and care transitions. Strong communication is essential.

Qualifications

• RN license is required.
• Bachelor's degree in Nursing highly preferred.
• Two to three years of related experience required; hospital and/or long-term care clinical experience highly preferred.
• Experience in healthcare marketing preferred.
• Strong written and verbal communication skills in English.

Responsibilities

• Attends staff meetings, educational programs, and community events to promote hospice services.
• Explains hospice services and benefits to patients, families, and significant others to foster positive views of hospice care.
• Facilitates admissions from hospital to home and coordinates transportation, DME, and medications.
• Works with hospitals and facilities to identify patients matching hospice care and to meet growth targets.
• Provides education to hospitals and facilities on the purpose and process of general inpatient care.
