The Care Team Home Health & Hospice is seeking a Hospice Clinical Liaison (RN/LPN) who thrives on community engagement and relationship-building. You will connect patients with appropriate hospice services by educating partners, coordinating referrals, and ensuring smooth transitions from referral to admission.

Ideal candidates are clinically confident, personable, and self-driven, able to build credibility with hospital staff and leadership.

Qualifications

• Active and unrestricted RN or LPN license required.
• Two or more years hospice, home health, geriatric care, or related healthcare experience preferred.
• Excellent communication, presentation, relationship-building, and customer service skills.

Responsabilités

• Build and maintain strong relationships with hospitals, physicians, skilled nursing facilities, and other referral partners.
• Provide education, consultation, and guidance on hospice eligibility, disease progression, and symptom management.
• Maintain a consistent presence within assigned facilities and support timely referral and admission coordination.
• Partner with clinical leadership, admissions, and operations to remove barriers and improve referral-to-admission conversion.
• Conduct clinical screenings and hospice eligibility assessments within RN scope of practice and document thoroughly.
