Job summary

Allied Services seeks a Hospice Liaison RN to coordinate referrals for inpatient hospice, home hospice and palliative care. You will evaluate admissions, complete required paperwork, and market services to referral sources to improve patient access.

Strong clinical assessment and communication with multidisciplinary teams are essential. The ideal candidate has a nursing degree, PA license, and at least five years of RN experience, with hospice background preferred and willingness to travel to

Qualifications

• RN license in good standing with the Pennsylvania Board of Nurse Examiners.
• Graduate of an accredited Nursing Program.
• Hospice experience preferred.
• Five years minimum RN experience; home or inpatient hospice preferred.

Responsibilities

• Evaluates patients and documents findings for admission decisions.
• Communicates goals and hospice information with patients and families.
• Meets with Medical Director to review candidacy for admission.
• Contacts Social Workers at referring facilities for patient abstracts and updates.
• Follows up with referred patients to reassess admission readiness.
