Job summary

The Care Team Hospice in Akron, OH is seeking an experienced Hospice Clinical Liaison (RN/LPN) to strengthen partnerships with hospitals, skilled nursing facilities, and physicians. This role bridges clinical expertise with referral development and ensures smooth transitions from referral to admission.

The ideal candidate is clinically confident, highly personable, and able to build credibility with leadership and nursing staff within assigned facilities while traveling across the region.

Qualifications

• Active and unrestricted RN or LPN license.
• Two+ years of hospice, palliative care, home health, SNF, geriatric care, or related healthcare experience preferred.
• Strong knowledge of hospice care, eligibility criteria, and Medicare Hospice Benefit.

Responsibilities

• Build and maintain relationships with hospitals, skilled nursing facilities, and other referral partners.
• Provide hospice education, clinical consultation, and guidance on eligibility, disease progression, and symptom management.
• Identify patients who may benefit from hospice services and support timely referral and admission coordination.
• Collaborate with care teams to remove barriers and improve referral-to-admission conversion.
• Conduct clinical screenings within RN scope of practice and ensure proper documentation.
