Job summary

The Care Team Hospice is seeking a personable Hospice Clinical Liaison (RN/LPN) to strengthen partnerships with hospitals, skilled nursing facilities, physicians, and other healthcare providers. You’ll work in the community, educating professionals and helping patients access hospice services, acting as a trusted clinical resource for referrals.

We value self-driven, outgoing nurses who can walk into facilities, connect with leadership, and foster relationships that ensure smooth transitions

Qualifications

• Active and unrestricted RN or LPN license in the state of practice.
• Previous nursing experience required; hospice, home health, palliative care, skilled nursing, or geriatric experience preferred.
• Strong communication, interpersonal, and relationship-building skills.
• Self-motivated, organized, and confident working independently in the community.
• Valid driver’s license, reliable transportation, and current automobile insurance.

Responsibilities

• Build and maintain relationships with hospitals, skilled nursing facilities, and other referral partners.
• Educate healthcare professionals on hospice eligibility, progression, and symptom management.
• Identify patients who may benefit from hospice services and facilitate referrals and admissions.
• Collaborate with clinical leadership and business development to improve communication and referral experiences.
• Conduct clinical screenings and eligibility assessments within scope while maintaining documentation.
