Job summary

Dignity Care Hospice Health in Independence, OH is seeking a Hospice Account Executive who will match service locations’ capabilities to patient needs and facilitate patient flow from acute care providers into Stonerise to meet census and goals.

You will communicate with the interdisciplinary team daily, manage the electronic referral system, develop relationships with case managers, discharge planners, hospital staff and families, and present the program with professionalism and care.

Qualifications

• Two years of experience in sales and marketing.
• Current and valid driver’s license with reliable transportation and ability to drive.
• Familiarity with electronic referral systems.

Responsibilities

• Match service locations’ capabilities to patient needs and facilitate patient flow from acute care providers into Stonerise.
• Manage the electronic referral system for assigned centers and build relationships with case managers, discharge planners, hospital staff and families.
• Coordinate clinical reviews via Allscripts and onsite evaluation to ensure seamless transitions to centers.
• Maintain visibility as the assigned hospital resource to promote Stonerise positively.
• Ensure HIPAA compliance and confidentiality in all activities.
