Heart to Heart Hospice seeks a Customer Service Liaison to strengthen our presence in hospitals, nursing homes, and home health settings. You will coordinate care among healthcare professionals, maintain weekly contact with patients and families, and help ensure a high level of satisfaction with our services.

The role requires 2–3 years of healthcare experience, a valid driver's license, reliable transportation, and familiarity with EMR systems; after-hours and weekend availability may be needed.

Formación

• 2-3 years of healthcare experience required.
• Excellent customer service skills required.
• Hospice, home health, nursing home, assisted living, or hospital experience preferred.
• Current State Driver’s License required.
• Must have reliable transportation and ongoing auto liability insurance
• Ability to establish and maintain effective working relationships with staff, facilities, public, Board of Directors and Medical Director.
• Demonstrates good communication and interpersonal skills.
• Experience with EMR software is a plus.

Responsabilidades

• Promote a customer-focused hospice presence in healthcare communities including facilities, home health, and hospitals.
• Coordinate care amongst healthcare professionals.
• Communicate weekly (daily if needed) with patient and family to ensure needs are met and high satisfaction.
• Identify and resolve patient/family dissatisfaction and inform clinical team and management as needed.
• Coordinate and provide in-services or educational events for family members and facilities.
• Work with agency leadership to meet growth and development targets.
