Job summary

Heart to Heart Hospice is seeking a Customer Service Liaison to promote compassionate care across facilities and hospitals. You will coordinate patient care with healthcare teams and maintain ongoing communication with patients and families to ensure high satisfaction.

The role requires strong healthcare exposure, excellent service skills, and the ability to work after hours. A valid driver’s license and reliable transportation support field duties and community engagement.

Qualifications

• 2-3 years of healthcare experience.
• Excellent customer service skills.
• Hospice, home health, nursing home, assisted living, or hospital experience preferred.
• Current State Driver’s License.
• Reliable transportation and ongoing auto liability insurance.
• Effective working relationships with staff, facilities, and leadership.
• Strong communication and interpersonal skills.
• Experience with EMR software is a plus.

Responsibilities

• Promote a customer-focused hospice presence in healthcare facilities and hospitals.
• Coordinate care amongst healthcare professionals.
• Communicate with patient and family weekly (daily if needed) to ensure satisfaction.
• Identify problems or dissatisfaction and ensure quick resolution with clinical team contact.
• Coordinate and provide in-services or educational events for families and facilities.
• Collaborate with agency leadership to meet growth targets.
• Provide after-hours or weekend consultations as needed.
• Ensure compliance with local, state, and federal regulations and policies.
