Heart to Heart Hospice in Texas is seeking a Customer Service Liaison to promote compassionate, patient-centered care across healthcare facilities and communities. You will coordinate care among professionals and serve as a key contact for patients and families to ensure satisfaction.

Qualified candidates will have 2-3 years in healthcare, excellent customer service skills, and the ability to build strong relationships.

Qualifications

• 2-3 years of healthcare experience.
• Excellent customer service skills.
• Ability to establish and maintain effective working relationships with staff and various stakeholders.
• Current State Driver’s License.
• Reliable transportation with ongoing auto liability insurance.
• Experience with EMR software is a plus.

Responsibilities

• Promote a patient-focused hospice presence in healthcare communities.
• Coordinate care among healthcare professionals.
• Communicate with patients and families regularly to ensure needs are met and satisfaction remains high.
• Identify problems or dissatisfaction and facilitate quick resolutions with clinical teams.
• Coordinate in-services or educational events for families or facilities.
• Work with leadership to meet growth and development targets.
• Flex to after-hours and weekend consultations as needed.
• Ensure compliance with laws, regulations, and company policies.
