Job summary

Heart to Heart Hospice is seeking a Customer Service Liaison to promote a patient-centered presence in the healthcare community within California, including assisted living facilities, nursing homes, home health, and hospitals.

You will coordinate care among healthcare professionals, communicate with patients and families, and ensure high levels of satisfaction while maintaining compliance with laws and policies.

Qualifications

• 2-3 years of healthcare experience
• Excellent customer service skills
• Experience with EMR software is a plus
• Current State Driver’s License is required
• Reliable transportation and auto liability insurance
• Ability to communicate with diverse stakeholders

Responsibilities

• Promote a customer-focused hospice presence in healthcare community (assisted living facilities, nursing homes, home health, hospitals)
• Coordinate care among healthcare professionals
• Communicate weekly (daily if needed) with patient and family to ensure needs are met and satisfaction levels are high
• Identify problems or dissatisfaction and ensure quick resolution with clinical team/management
• Coordinate and provide in-services or educational events for family members/facilities
• Work with agency leadership to meet growth/development targets
• Be flexible with after-hours and weekend consultations
• Ensure compliance with local, state and federal laws and Medicare regulations
