Heart to Heart Hospice seeks a Customer Service Liaison in Houston to promote hospice presence in the community. You will coordinate care across facilities and clinical teams, and maintain regular communication with patients and families to ensure high satisfaction.

Requirements include 2-3 years in healthcare, excellent customer service, a valid driver's license, and EMR experience is a plus. Role requires flexibility and commitment to after-hours and weekend availability.

Competenze

2-3 years of healthcare experience.
Excellent customer service skills.
Current State Driver’s License and reliable transportation.
Ability to communicate effectively with patients, families and staff.
Experience coordinating care across facilities and with clinical teams.

Mansioni

Promote a customer-focused hospice presence in healthcare facilities and hospitals.
Coordinate care among healthcare professionals and facilities.
Weekly (or daily) communications with patients and families to ensure needs are met.
Identify and resolve patient or family concerns promptly and relay issues to clinical teams.
Conduct in-services or educational events for families and facilities.
