Job summary

Heart to Heart Hospice in Indianapolis seeks a Customer Service Liaison to promote a patient-centered hospice presence and coordinate care across facilities, home health, and hospitals.

You will communicate with patients and families, resolve issues quickly, and work with leadership to meet growth targets; after-hours flexibility and travel may be required. Competitive benefits and 401k available.

Qualifications

• 2-3 years of healthcare experience.
• Excellent customer service skills.
• Hospice, home health, nursing home, assisted living, or hospital experience preferred.
• Current State Driver’s License.
• Reliable transportation with ongoing auto liability insurance.
• Able to establish and maintain effective working relationships with staff, facilities, and leadership.
• Demonstrates good communication and interpersonal skills.
• Experience with EMR software is a plus.

Responsibilities

• Promote a customer-focused hospice presence with facilities, home health and hospitals.
• Coordinate care among healthcare professionals.
• Communicate with patient and family regularly to ensure needs are met and high satisfaction.
• Identify problems or dissatisfaction and ensure quick resolution with the clinical team.
• Coordinate and provide in-services or educational events for family members and facilities.
• Work with agency leadership to meet growth and development targets.
• Flexibility in schedule for after-hours and weekend consultations.
• Ensure compliance with laws, Medicare regulations, and policies.
