Job summary

Gentiva Hospice is seeking a Hospice Care Consultant to educate providers, develop strategic referral partnerships, and drive early referral growth. You will build relationships with hospitals, home health agencies, SNFs, clinics, and physician offices to ensure the right patients receive hospice referrals at the right time.

Qualifications

• Bachelor’s degree or equivalent experience in a relevant field.
• Two+ years in healthcare sales, business development, or referral marketing.
• Strong relationships with physicians, hospitals, SNFs, or home health professionals.
• Experience in hospice, palliative care, or home health is a plus.
• Excellent communication, presentation and time-management skills.

Responsibilities

• Educate and inspire confidence in services to drive appropriate referrals.
• Identify new referral opportunities by engaging hospitals, SNFs, home health, clinics, and physician offices.
• Engage in conversations with healthcare professionals about patient needs and hospice options.
• Plan and execute marketing initiatives to increase awareness and referrals.
• Represent the organization at community events and professional associations.
