Gentiva Hospice is seeking a Hospice Care Consultant to champion our mission while developing strong referral partnerships across hospitals, SNFs, home health, clinics, and physician offices. You will educate providers, identify opportunities, and drive patient access to high-quality hospice care with a compassionate, strategic approach.

Qualifications

• Bachelor's degree or equivalent experience in a related field.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong relationships with physicians, hospitals, SNFs or home health pros.
• Hospice, palliative care, or home health experience a plus.
• Excellent communication, presentation, and time-management skills.
• Proficiency in MS Office and CRM platforms.
• Sensitivity to needs of terminally ill patients and families.
• Proven track record of meeting admissions and census targets.

Responsabilités
