Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• Experience in home care, hospice nursing, or public health preferred.
• First-hand ability to participate in quality assessment and improvement.
• Must possess dependability and ability to meet reporting requirements.

Responsibilities

• Plan, organize and direct hospice care for patients in homes and communities.
• Complete initial and ongoing comprehensive patient/family assessments.
• Develop and revise care plans based on nursing diagnosis and goals.
• Administer medications and treatments as prescribed.
• Provide health education and counseling to patients and families.
• Document clinical notes and communicate with physicians and team.
• Participate in on-call duties and ensure safe care environments.
• Meet regulatory and ethical reporting requirements.
