Job summary

Affinity Health Management is seeking a Registered Nurse to plan, organize and direct hospice care in home/community settings. The RN will assess patients, develop care plans, administer medications, and educate families, while collaborating with the interdisciplinary team to ensure compassionate end-of-life care.

The role requires current RN licensure, graduation from an NLN-accredited program, and prior experience in home health, hospice, or public health settings.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current RN licensure in state.
• One year of professional nursing experience in the last three years or a BSN with current RN license; two years in public health/home hospice preferred.
• Supervising hospice aides not required.

Responsibilities

• Provide comprehensive patient and family assessment to plan hospice care.
• Deliver professional nursing care using the nursing process.
• Develop and revise care plans with patient and family involvement.
• Administer medications and treatments as prescribed.
• Coordinate with the interdisciplinary team to meet patient needs.
• Participate in on-call duties and ensure equipment and services are available.
