Job summary

Affinity Health Management is seeking a Registered Nurse to plan, organize and direct hospice care in patients' homes and communities. The role emphasizes community health education, comprehensive patient assessments, and collaborative care planning with families and an interdisciplinary team.

The RN will provide care using the nursing process, administer prescribed medications, educate patients and families, and coordinate services while participating in on-call duties as required.

Qualifications

• Graduate of a National League for Nursing accredited school of nursing.
• Current licensure as a Registered Nurse in the applicable state.
• Minimum of 1 year RN experience within the last 3 years, or BSN with 2 years of experience including home health, public health or hospice.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process and plan of care.
• Develop and revise the plan of care with patient/family involvement as status changes.
• Administer medications and treatments as prescribed and educate patient/family on care needs.
• Act as case manager for assigned caseload when required.
• Maintain accurate clinical notes and communicate with physicians and care team.
• Participate in on-call duties and ensure availability of equipment and services.
