Job summary

Affinity Health Management in Bethlehem, PA is seeking a compassionate hospice aide to work under the supervision of a registered nurse, carrying out tasks per the patient’s plan of care to promote comfort and meet personal needs.

You will report observations to the RN Case Manager, assist with activities of daily living, monitor safety, aid in transfers, and help maintain a clean, respectful, and supportive home environment for patients and families.

Qualifications

• Completion of a state approved CNA course or equivalent training.
• NAC certification.
• Current Healthcare Provider CPR certification.
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and provides care to terminally ill.
• 1 year hospice/home health experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.

Responsibilities

• Perform personal care and attend to patient requests promptly.
• Provide communication to patient and family with confidentiality.
• Ensure safety using equipment and proper techniques.
• Assist with baths, grooming, oral hygiene, and changing bed linen.
• Assist in dressing and undressing as assigned.
• Assist with feeding while avoiding homemade foods.
• Monitor skin care to prevent tissue breakdown.
• Report pain or changes to RN Case Manager.
• Assist in ambulation and range of motion as instructed.
• Provide transfers and positioning as directed.
• Assist with medications reminders as allowed by state rules.
• Assist with bedpans, urinals, and light housekeeping.
• Perform simple procedures with competency as ordered.
• Provide respite care for patient/family when on-site.
• Keep living area clean and orderly.
• Follow documentation and care procedures and professional conduct.
• Participate in quality assessment and improvement activities.
• Respect patient and family environment and needs.
• As a mandated reporter report non-compliance and concerns as required.
• Report abandonment/abuse as required.
