Job summary

Affinity Hospice Management LLC seeks a compassionate hospice aide to perform personal care and comfort tasks under the supervision of a registered nurse, reporting observations and following the patient’s plan of care. The position emphasizes safety, cleanliness, and respectful interactions with patients and families.

The aide assists with daily activities, communicates clearly with the care team, and helps maintain accurate documentation and compliance with state regulations.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification.
• COVID-19 vaccination as per facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understanding hospice philosophy and comfort with terminally ill care.
• Prior experience in hospice, order ly, acute , rehabilitative, or long-term care; 1 year home health/hospice preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care tasks per patient assignment and respond promptly.
• Provide patient and family with respectful communication and confidentiality.
• Ensure safety and proper use of equipment (foot stools, rails, O2).
• Give baths, back rubs, oral hygiene, shampoos, and change bed linens.
• Assist with dressing and undressing as assigned.
• Assist feeding as directed; avoid homemade foods from the aide.
• Observe and report skin integrity to prevent breakdown.
• Report pain or changes to RN Case Manager; note caregiver issues.
• Assist in ambulation and exercise per nurse/therapist instruction.
• Provide ROM and positioning transfers as directed.
• Support self-administered medications as permitted by state rules.
• Offer bedpans/urinals and assist with light laundry and waste.
• Perform ROM and simple procedures with demonstrated skills.
• Provide respite for patient/family when on-site, as appropriate.
• Keep living area clean and orderly.
• Follow documentation and care procedures and maintain professional conduct.
• Participate in quality assessment and performance improvement activities.
• Respect patient and family environment and needs.
• As a mandatory reporter, report violations per state guidelines within 14 days.
• Report suspected abandonment or abuse to appropriate authorities immediately.
