Job summary

Affinity Health Management seeks a hospice aide to work under RN supervision, delivering compassionate personal care to patients in their homes or facilities. You will report observations, assist with daily activities, and help maintain a safe, clean environment in alignment with the patient’s plan of care.

The role emphasizes effective communication with patients and families, adherence to care procedures, and timely reporting of changes in patient condition to the RN Case Manager.

Qualifications

• State-approved CNA course or equivalent required.

Responsibilities

• Perform personal care tasks per patient assignment and report issues promptly.
• Provide compassionate communication with patient and family, preserving confidentiality.
• Ensure safety and proper use of equipment, e.g., transfer aids and rails.
• Assist with bathing, oral hygiene, grooming, and changing bed linens.
• Support in dressing/undressing and feeding as assigned.
• Assist with self-administered medications as allowed by state rules.
• Monitor skin condition to prevent pressure injuries and report changes.
• Document observations and care provided in EMR as required.
• Support patient mobility and positioning under supervision of RN/therapist.
• Provide respite to patient/caregiver when on-site and maintain clean living area.

Education

State-approved CNA course or equivalent

NAC certification

CPR certification

Food handlers permit

High school diploma or GED

Ability to read and follow written instructions

Hospice philosophy understanding

Prior hospice/long-term care experience

Basic EMR documentation skills

At least 18 years old

Licensed driver with insured vehicle
