Job summary

Affinity Health Management in Pennsylvania is seeking a Certified Nursing Assistant to work as a hospice aide under the supervision of a registered nurse. The role centers on comfort, observation, and documentation of care according to the patient’s plan.

You'll provide personal care, assist with transfers, monitor safety, and communicate with patients and families with compassion. Promptly report changes to the RN Case Manager and maintain dignity and safety at all times.

Qualifications

• Must have current PA CNA certification.
• High school diploma or GED required.
• Ability to read and follow care plans and document care.
• Previous hospice or long-term care experience preferred.
• Valid driver's license and reliable vehicle may be required.
• Must be fully vaccinated.

Responsibilities

• Provide personal care and assist with daily living activities.
• Communicate with patients and families with empathy and confidentiality.
• Ensure safety, proper use of equipment, and comfort.
• Assist with bathing, hygiene, dressing, feeding as assigned.
• Observe and report changes in condition to RN Case Manager.
• Assist with ambulation, transfers, and range of motion.
• Support with light housework and meal preparation as allowed.
• Maintain clean, orderly living areas and document care.
