Job summary

Affinity Hospice Management LLC is seeking a hospice aide to work under RN supervision, providing compassionate personal care to patients, supporting families, and ensuring safety as part of a patient-centered care plan.

Responsibilities include bathing, feeding assistance, documentation in EMR, and reporting changes to the RN Case Manager. Requires state CNA training, NAC, CPR, and a current Food Handlers Permit.

Qualifications

• Completion of a state-approved CNA course or equivalent.
• Nurse Aide Certification (NAC) currently active.
• Current CPR certification for Healthcare Providers.
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy and specialized end-of-life care.
• Prior hospice/home health/long-term care preferred (1 yr).
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.

Responsibilities

• Perform personal care tasks and respond to patient requests promptly.
• Provide compassionate communication with patient and family while maintaining confidentiality.
• Ensure safety and proper use of equipment (e.g., rails, O2).
• Assist with bathing, grooming, and changing bed linens as assigned.
• Assist with dressing, feeding, and daily activities per plan of care.
• Document observations and care provided; report changes to RN Case Manager.
• Assist in ambulation and range-of-motion as instructed.
• Support medications reminders within state regulations.
• Assist with light housekeeping and laundry; manage waste as assigned.
• Maintain clean, orderly living area and adhere to policies.
• Report any safety concerns or abuse/neglect per policy and law.
