Job summary

Affinity Health Management seeks a compassionate Hospice Aide to assist with personal care and comfort measures under RN supervision. You will report observations, follow care plans, and support families while ensuring safety and dignity for patients.

The role emphasizes respectful communication, adherence to policies, and accurate documentation in EMR. Prior CNA/NAC and CPR hands-on experience are preferred, with a valid Food Handler Permit and HS diploma or GED.

Qualifications

• Completion of a state-approved CNA/NAC course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Current Food Handler Permit.
• High school diploma or GED.

Responsibilities

• Assist with personal care needs and report observations to the RN Case Manager.
• Communicate with patients and families with confidentiality and respect.
• Ensure safety during mobility and use of equipment.
• Assist with bathing, oral hygiene, dressing, feeding, and linen changes.
• Monitor patient skin condition and report changes promptly.
• Record care provided and adhere to care plans and organizational procedures.
