Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification.
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill.
• Prior experience in home health/hospice preferred (1 year).
• Basic computer skills to document in electronic medical records (EMR).
• At least 18 years of age.
• Licensed driver with automobile insured and in good working order.

Responsibilities

• Perform personal care tasks as assigned and respond promptly to patient requests.
• Provide patient and family with respectful communication and maintain confidentiality.
• Ensure patient safety and use equipment properly.
• Assist with bathing, grooming, oral hygiene, and bed linens changes.
• Assist in dressing and undressing patients as needed.
• Assist in feeding as assigned while avoiding homemade foods.
• Monitor skin condition and report issues to RN Case Manager.
• Report changes in patient condition and pain to RN.
