Job summary

Affinity Hospice Management LLC is seeking a dedicated hospice aide to work under an RN, delivering compassionate personal care per the patient’s plan of care. You will report observations and care performed to ensure quality, continuity, and safety in the patient’s environment.

The role emphasizes respect for patient and family, safe use of equipment, and accurate documentation, with opportunities to collaborate in quality improvement activities within the hospice team.

Qualifications

• Completion of a state approved CNA program or equivalent.
• Florida NAC certification required.
• Current Healthcare Provider CPR with hands-on skills.
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Hospice philosophy understanding and comfort with terminally ill care.
• Prior hospice/home health/long-term care experience preferred (1 year).
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care tasks after assignment and attend to requests promptly.
• Provide patient and family with positive communication and maintain confidentiality.
• Meet safety needs and use equipment safely (foot stools, side rails, O2, etc.).
• Provide personal care including baths, back rubs, oral hygiene, and changing bed linens.
• Assist in dressing, undressing and feeding the patient as assigned.
• Assist with mobility, ambulation and transfers as directed by the nurse.
• Monitor pain or symptoms and report changes to the RN Case Manager.
• Keep patient area clean and orderly; support light housekeeping duties as needed.
• Document care provided in EMR and follow organization procedures.
