Job summary

GBMC HealthCare's Gilchrist Hospice Care is seeking a Hospice Admissions Nurse to assess patients at home or in hospitals and prepare them for entry into our spectrum of services. The role includes drafting initial care plans with Medical Director approval and providing direct nursing care to ensure comfort and eligibility.

The nurse serves as an ambassador for Gilchrist Hospice Care within the service area and collaborates with team leaders to coordinate resources and referrals.

Qualifications

• Knowledge of universal precautions and safe nursing standards.
• Strong time management with a changing daily schedule.
• Excellent interpersonal and communication skills.
• Ability to educate patients and caregivers.
• Proficient use of EMR systems.

Responsibilities

• Complete initial patient and family assessment for hospice eligibility and needs.
• Develop and implement initial care plan with Medical Director approval.
• Provide direct nursing care to maintain high-quality comfort.
• Inform leadership of resource utilization and community referrals.
• Document findings and participate in discharge planning with care teams.
• Educate family/caregivers on care tasks (medication, injections, IVs, dressings).
• Arrange home equipment and resources as needed.
• Support patients and families at time of death when applicable.
• Collaborate on admissions projects and process improvements.
• Build relationships with hospice referral sources and community representatives.
