Job summary

GBMC HealthCare seeks a Hospice Admissions Nurse to assess patients at home or in hospitals and prepare them for entry into the hospice services. This role acts as an ambassador for Gilchrist Hospice Care within the service area, coordinating with medical staff and community resources.

The ideal candidate will hold a Diploma/ADN/BSN and a current nursing license, with at least three years of nursing experience. Home health or hospice background is preferred, and EMR proficiency is required.

Qualifications

• Nurse must hold a nursing license and have relevant education.
• Excellent communication with patients and families is required.
• Able to manage changing daily schedules and prioritize care.
• Knowledge of EMR systems for charting and orders.
• Home health or hospice experience is preferred but not required.

Responsibilities

• Conduct initial patient/family assessment for hospice eligibility and needs.
• Develop and implement initial care plans with Medical Director approval.
• Provide direct nursing care to maintain high-quality outcomes.
• Coordinate with team and outside resources; document findings.
• Educate patients and caregivers on care techniques and medications.
