Job summary

GBMC HealthCare is seeking a Hospice Admissions Nurse to assess patients in their residence or hospital settings and prepare them for entry into Gilchrist Hospice Care services. The role serves as an ambassador for GBMC and Gilchrist within the service area.

The role requires a Nursing diploma/ADN/BSN, an active Maryland RN license, and 3 years of nursing experience (Home Health or Hospice preferred). EMR proficiency and strong communication are essential.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current MD nursing license.
• Three years of nursing experience; Home Health or Hospice experience preferred.

Responsibilities

• Perform initial patient and family assessment for hospice eligibility and needs.
• Develop and implement initial care plan with Medical Director approval.
• Provide direct nursing care to maintain high-quality patient care.
• Document clinical findings and participate in discharge planning with team members.
