Job summary

GBMC HealthCare is seeking a Hospice Admissions Nurse to assess patients in their residence or in hospitals and prepare entry into Gilchrist Hospice Care services. The role also serves as an ambassador for GBMC within the service area.

The nurse collaborates with the Admissions Team Manager to ensure compassionate, patient- and family-centered care, aligned with GBMC values and standards. Experience in home health or hospice is preferred but not required.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Active Maryland nursing license.
• Three years of nursing experience; home health or hospice preferred but not required.

Responsibilities

• Complete initial patient and family assessment to determine hospice eligibility and needs.
• Develop initial care plan for patient/family unit with Medical Director approval.
• Provide direct nursing care to maintain high-quality care.
• Inform leadership of resource utilization and referrals as appropriate.
• Document findings to support eligibility and discharge planning.
• Teach patients and family about care skills, pain control, meds, and safety.
• Arrange required home equipment and supplies.
• Attend to patient/family at time of death when applicable.
• Participate in department projects and process improvements.
• Build relationships with hospice referral sources in the service area.
