Job summary

Gilchrist Hospice Care seeks a compassionate Hospice Admissions Nurse to assess patients in their homes or hospitals and prepare them for entry into our spectrum of services. The role serves as an ambassador for Gilchrist Hospice Care within the service area and collaborates with the Admissions Team Manager.

Required are a Diploma/ADN/BSN, current MD nursing license, and at least three years of nursing experience (home health or hospice preferred).

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current MD nursing license.
• Three years of nursing experience; Home Health or Hospice experience preferred but not required.

Responsibilities

• Completes initial assessment of patient and family to determine Hospice eligibility and needs.
• Prepares and implements the initial care plan for patient/family with Medical Director approval.
• Provides direct nursing care to patients to maintain highest level of care.
• Documents clinical findings to ensure hospice eligibility and participate in discharge planning.
• Educates family members or hired caregivers in patient care skills (pain control, injections, IVs, dressing changes, hygiene, vital signs).
• Arranges home equipment and resources as needed; participates in admissions projects and process improvements.
