GBMC HealthCare is seeking a Hospice Admissions Nurse to assess patients in their home or hospital and coordinate entry into Gilchrist Hospice Care services. You will also represent GBMC HealthCare in the community and collaborate with the Admissions team to develop care plans.

The role requires a nursing license, ADN/BSN, and three years' experience with strong communication, time management, and EMR skills. Travel to patient residences may be required.

Qualifications

• Current MD nursing license is required.
• Diploma, ADN and/or BSN from an accredited institution.
• Three years of nursing experience; Home Health or Hospice experience preferred but not required.

Responsabilités

• Completes initial assessment of patient and family/caregivers to ascertain Hospice eligibility and needs.
• Prepares and implements the initial care plan for patient/family unit with Medical Director approval.
• Provides direct nursing care to patients to maintain highest level of care.
• Inform Team Leader of utilization of other team members, outside agencies, and community resources.
• Documents clinical findings to assure initial hospice eligibility and participate in discharge planning.
