GBMC HealthCare seeks a Hospice Admissions Nurse to assess patients in homes or hospitals, prepare them for admission, and act as an ambassador in the service area. Responsibilities include initial assessments, care planning, direct nursing care, and coordinating with teams and resources.

The role requires a Diploma/ADN/BSN, current RN license, and 3 years of nursing experience, with hospice/home health preferred. Excellent communication and EMR skills are essential.

Competenze

• Diploma, ADN and/or BSN from an accredited institution.
• Current MD nursing license.
• Three years of nursing experience; Home Health or Hospice preferred but not required.

Mansioni

• Completes initial assessment of patient and family/caregivers to ascertain Hospice eligibility and needs.
• Prepares and implements the initial care plan for patient/family unit with Medical Director approval.
• Provides direct nursing care to patients to maintain highest level of care.
• Inform Team Leader of utilization of other team members, outside agencies, and community resources upon initial assessment findings.
• Documents clinical findings to assure hospice eligibility and participate in discharge planning with relevant departments.
• Teaches family members or hired caregivers patient care skills and education.
