Greater Baltimore Medical Center, Inc. seeks a Hospice Admissions Nurse to assess patients in their homes or hospitals and prepare them for hospice services. This role also represents Gilchrist Hospice Care within the service area.

The position requires an RN license in Maryland, a diploma/ADN/BSN, and at least three years of nursing experience, with hospice/home health preferred. Competitive salary and benefits are offered as part of GBMC HealthCare.

Competenze

• RN license in MD required.
• Three years of nursing experience; home health or hospice preferred.
• Diploma, ADN and/or BSN from an accredited institution.

Mansioni

• Complete initial patient and family assessment for hospice eligibility and needs.
• Develop and implement initial care plan with Medical Director approval.
• Provide direct nursing care to maintain high quality care.
• Document findings and coordinate discharge planning with care team and social services.
• Educate patient and family on care skills and pain management.
• Ensure home equipment arrangements are in place and manage medications and IVs.
