GBMC HealthCare's Gilchrist Hospice Care in Towson, MD seeks a Hospice Admissions Registered Nurse to assess patients at home or hospital and prepare entry into our services. Requires Diploma/ADN/BSN, MD nursing license, and 3 years nursing experience; home health or hospice experience preferred.

You will coordinate care plans, educate families, and collaborate with the admissions team to ensure smooth transitions into Gilchrist Hospice Care's service offerings.

Qualifikationen

• ADN/BSN or higher from an accredited program.
• MD nursing license in good standing.
• Minimum of 3 years nursing experience.
• Home health or hospice experience preferred but not required.

Aufgaben

• Complete initial patient/family assessment for hospice eligibility and needs.
• Develop and implement initial care plan with Medical Director approval.
• Provide direct nursing care to maintain high-quality patient care.
• Coordinate with team, outside agencies and resources.
• Document findings and participate in discharge planning as needed.
• Educate family/caregivers on care skills and safety.
