Job summary

Gilchrist Hospice Care seeks a Hospice Admissions Nurse to assess patients in their home or hospital and prepare them for entry into the hospice program. The nurse will serve as an ambassador for Gilchrist Hospice Care within the service area and work with the Admissions team to coordinate care planning.

The role requires a Diploma/ADN/BSN, a current Maryland nursing license, and at least three years of nursing experience (home health or hospice preferred).

Qualifications

• Three years of nursing experience.
• Current Maryland nursing license.
• Diploma, ADN and/or BSN from an accredited institution.
• Home Health or Hospice experience preferred but not required.

Responsibilities

• Complete initial assessment of patient and family to determine hospice eligibility and needs.
• Prepare and implement the initial care plan with Medical Director approval.
• Provide direct nursing care to maintain highest level of care.
• Inform Team Leader of utilization of other team members and resources.
• Document clinical findings and participate in discharge planning with care teams.
