Job summary

Advocate Health in the South Team (Chicago Metro, South and SW suburbs) seeks a Part-time Hospice Admissions Coordinator to lead the interdisciplinary care team and manage patient admissions to hospice services.

You will obtain history, assess eligibility, discuss hospice options with families, review insurance benefits, and coordinate care planning with the IDT. A valid driver’s license is required; weekend and holiday rotation apply.

Qualifications

• A valid driver's license issued by the Division of Motor Vehicles.

Responsibilities

• Responsible for the admission to hospice services and leads the interdisciplinary care team for each patient and family on their caseload.
• Obtains history and completes physical assessment, collaborates with the attending physician and hospice IDT to determine level of care.
• Meets with patients and families to explain hospice services and obtain informed consent for hospice care.
• Reviews insurance/Medicare eligibility and benefits to support billing efforts.
• Assesses patient/family readiness and develops the plan of care with participation from the family.
• Coordinates services with the interdisciplinary team and arranges equipment and supplies as needed.
• Facilitates appropriate placement in hospice contracted facilities if necessary.
• Assists with delivery of medications and home medical equipment.
• Develops hospice education for inpatient staff and acts as an expert resource on hospice care.
