Advocate Aurora Health at Home is seeking a PRN Hospice Nurse Liaison for the South Chicago Market. The role supports patient transitions, coordinates with physicians and care teams, and educates patients on hospice services. Requires RN license and 2+ years in discharge planning or care management.

Position is part-time with 8–16 hours weekly, Monday–Friday, no weekend requirements, and a valid driver's license with reliable transportation.

Qualifications

• Registered Nurse license required.
• BSN preferred.
• 2 years of clinical nursing experience with discharge planning and/or care management.

Responsibilities

• Collaborates with physicians, nurses, social workers, care managers and community partners to facilitate smooth patient transitions.
• Attend huddles, discharge rounds, and meetings; serve as a resource on home-based hospice services.
• Educate patients on hospice eligibility and plan of care at discharge.
• Collect and document patient information to initiate Advocate Aurora Health at Home services.
• Educate patients and providers about financial coverage for services.
