Advocate Health Care is seeking a Hospice Admissions Nurse to serve the Chicago Metro area. The role is 32 hours per week with evenings, Monday/Tuesday/Thursday/Friday, 2:00 PM - 10:00 PM. Travel between locations is required and weekend/holiday rotations apply.

Strong assessment, communication, and coordination with the interdisciplinary team are essential. Required: RN license in Illinois, BLS, and a valid driver's license.

Qualifications

• RN licensure in the state of practice.
• BLS for Healthcare Providers certification.
• Valid driver's license; willingness to travel within service area.

Responsabilités

• Admit patients to hospice services.
• Lead the interdisciplinary team for each patient and family.
• Obtain patient history and perform physical assessment.
• Assess eligibility for hospice and level of care.
• Explain hospice services and obtain informed consent.
• Coordinate services with IDT and communicate handoffs.
