Advocate Aurora Health at Home in Illinois is seeking a part-time RN to promote hospice services at home. The role covers the South Chicago Market with 8–16 hours per week, Monday to Friday daytime shifts, no weekends or holidays required.

The position involves travel, education of patients and coordination with hospital and post-acute providers. Required are an active RN license in Illinois and reliable transportation; hospice experience is preferred.

Qualifications

• Licensed RN with valid Illinois license.
• BSN preferred.
• Driver's license and reliable transportation required.
• Hospice experience preferred.
• Ability to travel within the South Chicago Market.

Responsibilities

• Collaborates with physicians, nurses, social workers and care managers to facilitate smooth patient transitions.
• Attends huddles, discharge rounds and team meetings; serves as a resource for home-based services.
• Educates patients on hospice eligibility and ensure plan of care meets regulatory requirements.
• Collects medical, social and reimbursement data to initiate Advocate Aurora Health at Home services.
• Educates patient and providers about financial coverage and responsibility for services.
• May follow hospice patients readmitted to hospital or post-acute settings to assess continued needs.
• Supports admission growth and market share with the business development team.
• May participate in precepting new hire training and orientation.
