Job summary

Advocate Aurora Health at Home is seeking a PRN, part-time RN to support Hospice services in the North Chicago Market. Mon-Fri 8:30am-5:00pm with 8–16 hours per week; no weekends or holidays. Valid driver's license and reliable transportation are required. Hospice or sales experience is preferred.

The role involves educating patients and families, coordinating with physicians and care managers, and ensuring care plans meet hospice eligibility. Travel to assigned facilities will be needed.

Qualifications

• RN license required or in-state equivalent.
• 2 years of clinical nursing experience with discharge planning and/or care management.
• Hospice experience preferred.

Responsibilities

• Collaborates with physicians, nurses, social workers and care managers to facilitate patient transitions at home.
• Educates patients about hospice services and ensures plan of care meets eligibility and regulatory requirements.
• Manages referrals and coordinates with home health services to support hospice, infusion, enteral, pediatric, DME and RT services.
