Job summary

Advocate Health in the Chicago area seeks a Hospice Admissions Nurse (32 hours/week) to coordinate hospice admissions, assess patients, and work with the interdisciplinary team to ensure seamless care.

The ideal candidate is a compassionate RN with strong communication and assessment skills, able to travel between locations and support families through the admission process. 32-hour workweek with evening shifts and weekend rotation.

Qualifications

• Requires RN licensure in state of practice.
• Associate's Degree in Nursing is required.

Responsibilities

• Responsible for admission to hospice services and leads the interdisciplinary care team for each patient/family.
• Obtain history and complete assessments to determine appropriateness and level of care; collaborate with physicians and IDT.
• Explain hospice services, obtain informed consent, verify prognosis, review insurance eligibility, and support billing.
