Job summary

Carolinas Medical Center is seeking a compassionate Hospice Admissions Nurse for the Chicago Metro area. This full-time 32-hour/week role requires coordinating admissions, assessing patients, and guiding families through hospice services.

The ideal RN will have strong assessment, communication, and care coordination skills, with travel between locations and a 2+ year direct patient care background. No on-call required; some holidays and weekends rotation apply.

Qualifications

• Must be a registered nurse with state licensure.
• BLS for Healthcare Providers required or obtainable within 6 months.
• Ability to travel between Advocate Aurora locations in service area.
• Excellent communication and interpersonal skills.
• Strong assessment, documentation and time management abilities.
• Experience in hospice or palliative care is a plus.

Responsibilities

• Lead the admission process for hospice services and coordinate the IDT for each patient.
• Obtain patient history, perform physical assessment, and evaluate eligibility for hospice care.
• Explain hospice services to patients/families and obtain informed consent; certify prognosis with IDT.
• Review insurance/Medicare eligibility and assist billing documentation.
• Assess readiness and develop plan of care with patient/family involvement.
• Coordinate services with IDT and facilitate appropriate facility placements when needed.
