Job summary

Advocate Health is seeking a Hospice Registered Nurse in the Chicago, IL area to lead the interdisciplinary care team for each patient and family on their caseload. You will assess needs, coordinate services, obtain informed consent, and ensure appropriate hospice enrollment and documentation.

The role requires an RN license, BLS, and an ability to travel within Advocate Aurora service areas. Two years of direct patient care is typical, with emphasis on palliative care and thorough communication.

Qualifications

• Registered Nurse license in the practicing state.
• BLS for Healthcare Providers certification (AHA) may be required.
• Valid driver’s license with ability to travel within service area.

Responsibilities

• Lead the interdisciplinary care team for each patient and family.
• Assess patient and family needs and determine appropriateness for hospice services and level of care.
• Explain hospice program and obtain informed consent; verify prognosis with IDT.
• Review insurance/Medicare eligibility and assist with billing documentation.
• Coordinate services with IDT and arrange supplies, equipment, and home services.
• Facilitate appropriate placement in hospice facilities when needed.
• Coordinate medication, supplies, and home medical equipment delivery.
• Provide hospice education to inpatient hospital and facility staff.
