Job summary

Advocate Health is seeking a skilled Hospice Admissions Nurse for our Illinois locations. This full-time role covers evening shifts, with 12:00 PM-8:00 PM or 2:00 PM-10:00 PM options and rotation of weekends and holidays.

You will lead the hospice admission process, assess patients, coordinate care with the IDT, and ensure seamless communication with families. Travel across Advocate Aurora service areas is required.

Qualifications

• Registered Nurse (RN) license in the state practicing.
• BLS for Healthcare Providers certification (AHA)
• Valid driver’s license

Responsibilities

• Lead admission to hospice services and coordinate with the IDT.
• Obtain patient history and complete physical assessments for hospice eligibility.
• Explain hospice services and obtain informed consent; verify prognosis with IDT.
• Review insurance/Medicare eligibility for billing support.
• Assess patient/family readiness and develop care plans with referrals as needed.
• Coordinate care with the interdisciplinary team and arrange equipment or supplies.
• Facilitate placement in contracted hospice facilities when needed.
• Assess and coordinate delivery of medications and home medical equipment.
