Zusammenfassung

The Hospice Admissions Nurse reports to the Admissions Team Manager. The nurse assesses patients in their place of residence or in hospitals and prepares the patient for entry into the spectrum of services provided by Gilchrist Hospice Care. The Hospice Admissions Nurse also represents Gilchrist Hospice Care within the service area.

Education: Diploma, ADN and/or BSN from an accredited institution. Licensure: Current RN nursing license.

Qualifikationen

• Nursing degree required (Diploma/ADN/BSN).
• Current RN license. Three years of nursing experience.

Aufgaben

• Assess patients in home or hospital to determine hospice eligibility and needs.
• Prepare initial care plan with Medical Director approval.
• Provide direct nursing care to maintain high-quality care.
• Document findings for eligibility and discharge planning.
• Educate patient/family on care techniques and medications.
