Job summary

Chapters Health seeks an Admission RN to assess patients in their residence or hospital and initiate care within the Chapters Health System. You will serve as an ambassador of the organization, ensuring a seamless transition to Care Navigation and coordinating early patient needs.

The role requires RN licensure in the working state, BLS, and at least one year of RN experience (hospice or hospital preferred).

Qualifications

• Current RN license in the state where employed.
• Active BLS certification from AHA or Red Cross.
• Minimum 1 year RN experience; hospice or hospital experience preferred.
• Strong clinical assessment skills; background in ED or ICU preferred.
• Experience with EMR/EHR systems.
• Ability to communicate in English; Spanish preferred.
