Chapters Health System seeks an Admission RN to assess patients at their residence or hospital, initiating care and ensuring a seamless transition to Care Navigation. You’ll act as an ambassador for the organization and coordinate with care teams.

Ideal candidates hold an active RN license, BLS, and at least one year of RN experience, with hospice or hospital background preferred; English communication is essential, Spanish preferred, with strong clinical assessment and multitasking abilities.

Formación

• Current RN license in the state of employment.
• Active BLS for healthcare professionals from AHA or Red Cross.
• Minimum of one year RN experience; hospice or hospital experience preferred.
• Experience working with an EMR/EHR system.
• Ability to communicate effectively in English (verbal and written); bilingual in Spanish preferred.
• Valid driver’s license and automobile insurance per policy; reliable transportation.
• Ability to lift and move patients and/or equipment; meet physical requirements.

Responsabilidades

• Completes clinical admissions for all company services; explains services to patients/families and addresses questions with empathy.
• Educates patient/family on admission requirements and initial assessments.
• Initiates Plan of Care and physician orders; initiates nursing interventions to prevent complications.
• Acts as company representative at facilities and coordinates with hospitals, physicians, and care teams.
• Ensures timely follow-up with Care Navigation; uses SBAR for handoffs; arranges safe discharge when appropriate.
